About the Role
We are seeking an Intensive Case Manager to join Amity Foundation's prison programs and re-entry services in Los Angeles County. This role focuses on creating supportive networks of services and resources for clients to prevent reincarceration and support long-term permanent housing goals.
Key Responsibilities
- →Interview and assess participant needs through referrals from the Office of Diversion and Reentry
- →Conduct wellness checks at clients' homes and provide crisis management
- →Develop treatment plans with participants to identify and address service needs and goals
- →Assist with public benefits advocacy and linkage
- →Coordinate and link clients to mental health, substance abuse, and housing resources
- →Work with staff to assist with placement and case management upon release
- →Collaborate with housing staff, probation officers, psychiatrists, public defenders, and other partners
- →Attend inter-agency case meetings, coalitions, and advocacy groups
- →Maintain concise and comprehensive case records on all participants
- →Provide accurate daily and monthly documentation as per funding guidelines
- →Use company and personal vehicles to visit county jails and transport clients
Requirements
- ✓Bachelor's degree in behavioral health or social science with one year of outreach/substance abuse counseling experience
- ✓OR high school diploma with three years of administrative responsibility in outreach, substance abuse counseling, reentry services, and case management
- ✓Knowledge of administering assessments, substance abuse counseling, reentry services, and case management within culturally diverse populations
- ✓Excellent organizational skills and attention to detail
- ✓Excellent written and oral communication skills
- ✓Extensive knowledge of non-profit organizations
- ✓Proficient in Microsoft Office Suite or similar software
- ✓Valid driver's license, vehicle, and car insurance
